Hazel Olean Polk 76, of Crestview, Florida passed away on Friday, February 16, 2024. Hazel was born on February 21, 1947 in McKenzie, Alabama to Ois and Lillian Browder.
Hazel grew up in Crestview and met the love of her life Johnny. They married and started their family. Hazel was a loving mother and wife. She supported her husband and his construction business. She also was an Okaloosa County School bus driver for many years. She had a love for the outdoors including fishing and hunting. She would rather be hunting, and her wall display demonstrates her success. Hazel was proud of all her children and grandchildren and instilled in them the love of outdoors as well. Hazel never met a stranger and always welcomed family and friends into her home.
Hazel is preceded in death by: Father, Ois Browder, Mother, Lillian Browder, Father in-law Mr. Bill Polk, Mother in-law, Othene Polk, sister, Lillian Rogers, brothers, Cody and Paul Browder, granddaughter, Robin Nicole Middlebrooks.
Survived by: Husband: John W. Polk Jr. of 60 years, and her beloved fur companion “prissy”
Son: Jeff Polk (wife Mandy)
Daughters: Sherry Strickland, Tammy Polk, Cindy Bell (husband Kevin)
Sister: Nita Miller
Grandchildren: Shannon Reed (husband Mathew), Jonathan Brooks, Tristin Barry (husband Jim), Cameron Thomas (fiance Patti), Ashley Hale (husband Andrew), Austin Polk (wife Shelby), Zack deGraaf (wife Natalie), Carson Polk.
Great Grandchildren: Haleigh, Maddie, Wyatt, Amrielle, Kelsie, Khloe, Seth, Kobi, Tommy, Mattie, Ella, CJ, Ellie Grace.
Sister in-law Dale Shirey (husband Jim), Carla Polk, and numerous nieces & nephews.
A funeral service will be held on Wednesday, February 21, 2024 beginning at 6:oo p.m. from The Chapel of Brackney Funeral Service with Mr. Kevin Casey speaking. The family will receive friends on Wednesday, February 21, 2024 from 5:00 p.m. – 6:00 p.m. at the funeral home. * Please wear Camo, if you wish in honor of Momma.
