Sheila Ann Howard Forman 66, of Crestview, Florida passed away on Monday, November 13, 2023 peacefully at home with her family at her side.
Sheila was born on November 19, 1956 in Crestview, Florida to Theodore and Irene Howard.
Sheila was born and raised in Crestview, Florida, she was a 1975 graduate of Crestview High School and after graduation she moved to Tallahassee, Florida where she was a secretary for the State of Florida. Then after several years she moved back to her hometown of Crestview where she was a resident ever since. Sheila was a member of Victorious Life Worship Center who enjoyed watching cooking shows then prepare what she learned from the show and made fabulous meals.
Sheila is preceded in death by her parents, T.H. and Irene Howard, her two brothers, Paul Lamar Howard and James Earl Howard, her half brother, Eulis McKinley Howard and her sister, Carol Jane Howard Hayes.
Sheila is survived by her two children, Shannah Nicole Forman and Megan Kayla Forman, her three sisters, Ruth (Gene) Stanton, Gayle (Jimmy) Hobbs and Kem Sellers, her seven grandchildren, Faith, Amber, Hayden, Dixie, Jack, Ryan and Luke, her lifelong friend, Vicki Pelt and numerous nieces and nephews.
A funeral service will be held on Friday, November 17, 2023 beginning at 2:00 p.m. from the Chapel of Brackney Funeral Service with Pastor Donnie Cadenhead officiating. Burial to follow at Live Oak Park Memorial Cemetery in Crestview, Florida. The family will receive friends on Friday, November 17, 2023 from 1:00 pm. – 2:00 p.m. at the funeral home.
